

Welcome to our 2026 Innovation Incubator Cohort
The National Association of Community Health Centers (NACHC) is pleased to announce the 8 health centers selected for the 2026 Innovation Incubator cohort! This year’s program is focused on one of the most pressing challenges: Financial Diversification & Sustainability. In an era of shifting payer models and rising operational costs, these health center innovators are stepping up to design, pilot, and test solutions that will secure the long-term viability of the Health Center movement.

- Minnesota and North Dakota: Community Health Service, Inc.
- Massachusetts: Family Health Center of Worcester
- Texas: AccessHealth
- Indiana: HealthLinc
- Missouri: Hope Family Care Center
- Florida: MCR Health
- Nebraska: One World Community Health Centers
- Hawaii: Waianae Coast Comprehensive Health Center

Each health center will receive $30,000 in seed funding, along with intensive coaching in human-centered design. Their projects will tackle critical areas such as expanding reimbursement beyond PPS rates, developing new non-required services, and establishing sustainable subsidiary business models. We know these topics are a priority, as over 75 health centers submitted applications for the cohort.
The goal of the Incubator isn’t just to support these eight health centers- it’s to create a blueprint for sustainability for the entire 1,500+ health center network! We can’t wait to share their progress
