Family Healthy Weight Implementation Basics

Overview
FHWP Community of Practice
The National Association of Community Health Centers (NACHC) will host a virtual Community of Practice that will introduce participants to the Family Healthy Weight Program approach to the prevention and treatment of pediatric obesity, with a family-centered lens. This series will guide participants through key elements of the Family Healthy Weight Program model for real-world application.
Each session will highlight 2-3 steps for implementation of an evidence-based program, based on the newly produced Family Healthy Weight Implementation Guide and Microlearning Series. This virtual series will provide participants with actionable strategies for program implementation and sustainability.
Learning Objectives
By the end of the Community of Practice, participants will:
- Increase knowledge and awareness of the components of a Family Healthy Weight Program
- Identify key steps for implementing an evidence-based pediatric weight management program
- Assess program success through evaluation methods aligned with program goals
- Address resource capacity and sustainability for long-term integration
Requirements
- Participating Health Centers must agree to attend each of the four sessions by having a team member present to represent their center.
- Each participating Health Center must submit an application to register for the webinar series
Community of Practice Structure
- The Community of Practice will be organized into four (4) sessions. Each session will be 60 minutes using Zoom as the platform
