Food for Health Promising Practices: Clinical Integration
This webinar is part of a three-part spring series under NACHC’s Abbott-funded Food for Health initiative, highlighting promising practices from community health centers. Each session aligns with one of the Food for Health pillars and features peer-led examples that demonstrate practical implementation, sustainability considerations, and policy relevance.
This session focuses on the Clinical Integration pillar and will highlight how health centers are integrating Food for Health into primary care workflows, including nutrition screening and referral processes, EHR workflows, partnerships with food banks and community organizations, and billing and reimbursement strategies. The session will also connect practice-based lessons to federal policy considerations.

Food for Health Promising Practices: Expanding Food Access and Security through CHC Partnerships
This session will highlight how community health centers are expanding food access and addressing food insecurity through cross-sector partnerships and community-based strategies. Speakers will share practical examples of food distribution models, partnerships with food banks and local organizations, and approaches to improving access to healthy food for medically underserved populations. The session will also highlight lessons learned and considerations for sustainability that can support replication across health centers.

Food for Health Promising Practices: Supporting Nutritional and Lifestyle Approaches at the Individual Level
This session will focus on how community health centers support patient-centered nutrition education and lifestyle changes that promote long-term health. Speakers will share examples of programs such as cooking literacy, nutrition coaching, peer support groups, and diabetes self-management initiatives designed to empower patients and improve chronic disease outcomes. The session will also highlight culturally responsive approaches and practical strategies that health centers can adapt within their own communities.
