This session explores how Primary Care Associations (PCAs) and Health Center Controlled Networks (HCCNs) work together to address childhood chronic diseases at scale, with PCAs providing workforce training, technical assistance, and policy alignment while HCCNs deliver data analytics, interoperability support, and rapid-cycle quality improvement. Participants will learn practical regional collaboration strategies for leveraging these complementary resources to improve outcomes for children with chronic conditions and sustain innovations across community health settings.
This webinar is co-hosted by NACHC and MHP Salud through the Connected Communities Health Initiative (CCHI). As part of the National Technical Assistance Partner (NTAP) network, CCHI collaborates with health centers nationwide to strengthen the health and wellbeing of patients and communities. Through this partnership, practical tools, actionable resources, and engaging learning opportunities are offered to equip providers to address non-clinical factors of health and drive meaningful, lasting change.
