Overview
Target: BP™ is a national initiative created by the American Heart Association (AHA) and the American Medical Association (AMA) in response to the high prevalence of uncontrolled blood pressure (BP). Committed to advancing positive health outcomes, Target: BP supports health care organizations and communities to improve blood pressure control by:
- Leveraging AHA guidelines and scientific statements and the AMA MAP™ framework
- Assisting health care organizations in their quality improvement (QI) journeys to improve and sustain BP control
- Recognizing health care organizations annually with achievement awards
Learning Objectives
This workshop would provide an overview of the key concepts of the AMA MAP framework as an approach to hypertension QI including measuring accurately, acting rapidly, and partnering with patients. Available resources associated with each aspect of the framework will be highlighted to strengthen professional competency, systematic care processes, and culturally relevant patient education efforts. In addition, the structure, criteria, process, and benefits of submitting data for annual achievement awards will be discussed using aggregated performance data from the 839 FQHCs currently participating in Target: BP.
Award Criteria
The award criteria emphasize the adoption of evidence-based activities associated with the MAP framework, including self-measured blood pressure monitoring and equitable health outcomes. Participants can use assessment tools and the award criteria to evaluate their current practices, identify gaps, choose improvement resources, and take action to improve blood pressure control. Examples will be shared illustrating how annual awards can be used to incentivize improvement, celebrate team-based efforts, and promote organizational excellence with patient and community members.
