The Candidates’ Forum provides an opportunity to learn more about the individuals running for election at the 2025 House of Delegates Annual Meeting. Each candidate has submitted a personal statement outlining their experience, leadership vision, and commitment to the health center movement. We encourage you to take a moment to read through each statement to make an informed decision in the upcoming election.
There are seven (7) candidates running for the 2025 House of Delegates. Only the Health Center Board Member Representative seat is contested, with three (3) candidates.
NACHC members are encouraged to meet and visit with all candidates prior to the election. Space will be designated near Registration for each candidate campaigning for NACHC office. Campaign signage and literature may not be posted or displayed anywhere in the Hyatt Regency Chicago, including lounge areas, registration area, exhibit hall, or any other conference venue. Hotel management strictly prohibits the affixing of signage to walls or structures within its building(s).
Candidates:
Chair-Elect: Felix Valbuena, Jr., MD

For the past 36 years, I’ve had the honor of serving the Community Health and Social Services
(CHASS) Center, an FQHC proudly rooted in Southwest Detroit and now celebrating 55 years of
service. My journey—from social worker and community resident to family physician, Chief
Medical Officer, and now Chief Executive Officer—has been deeply grounded in the values that
define our health center movement: equity, access, and community empowerment.
At CHASS, we’ve led the shift toward value-based care by integrating primary care, behavioral
health, dental, and enabling services under one roof—improving patient outcomes while
addressing the social drivers of health. Our enduring partnership with a major healthcare system
exemplifies the kind of collaborative, community-focused innovation that defines the future of
FQHCs.
Nationally, I’ve remained committed to advocacy and strategic leadership through my roles with
NACHC. I’ve served on the Health Policy Committee (2008–2013, 2016–present) and currently
as Chair, the Health Professions Education in Health Centers Task Force (2016–present), and
the Clinical Practice Committee since 2008, including as Chair from 2016 to 2023. These experiences have reinforced my belief in the power of strong, clinician-informed leadership to
drive transformative policy and payment reforms.
As Chair-Elect, I am committed to elevating the unified voice of our FQHC network, advancing
policies that protect our funding, supporting workforce development, and expanding valuebased models that reward quality, not just volume. I am committed to leveraging our Strategic
Framework to continue driving our efforts to be the employer, provider, and partner of choice,
ultimately reaching 1 in 3 lives in the U.S. I respectfully ask for your support so that, together,
we can continue building a sustainable, equitable, and resilient future for the communities we
serve.
Speaker: Kimberly Chang, MD

I humbly ask for your support to serve as Speaker of the House. I’ve contributed on the board of NACHC as Speaker for the last two years and as Vice Speaker for the prior five. I’ve committed my entire professional career of 23 years to service in CHCs, and have always promoted our patients and our health centers. The primary reason I’ve stayed in health centers is because of the unity of CHCs as a national movement, with NACHC leading the way. I’ve served on and co-chaired many committees for NACHC, wrote blogposts and advocated, developed/moderated/facilitated conference sessions/meetings, and uplifted health centers at every turn. Outside of NACHC-specific activities, I see patients at Asian Health Services, in Oakland, California. I helped develop and am faculty for Health Partners on IPV + Exploitation, an NTTAP supporting CHCs to improve access and care to patients at risk of, experiencing, or surviving intimate partner violence, human trafficking, and exploitation. This work, based on the foundation of patients’ I’ve cared for, has brought me to Primary Care Associations, and CHCs across the nation and Western Pacific.
I’ve been thrilled to share knowledge and build upon common experiences and mission with you. I further elevate our patients and CHCs through academic writing (peer-reviewed articles and textbook chapters) and invited Grand Rounds and lectures throughout the country, particularly to help catalyze the interest of potential workforce providers to join CHCs nationwide. I have also served on several national federal advisory committees and commissions, all with the intention to uplift the experience of our patients and health centers. As Speaker, I will honor our history of patient-centered care and advocacy, and forward our collective goal of continuing to be the best primary care system in the United States!

Kimberly Chang
Treasurer: Eleace Sawyers, PhD

I am honored to be nominated for NACHC’s Treasurer. This position comes with great responsibility to NACHC and its member organizations, and requires in-depth knowledge of the NACHC, as well as the experience, judgement, ethical character, equitable approach, and transparency that are essential for managing the NACHC’s finances. Moreover, in the currently uncertain financial times, serving as Treasurer requires vision for creating novel approaches to maximize the NACHC’s financial assets.
I bring the above qualities to the position of Treasurer. I have been actively attending NACHC’s initiatives and training since 2005, joined NACHC as an individual member in 2016. Through, now, three community health centers involvement as a staff, I have continued some involvement with NACHC’s offering up to the present.
My past and present committee involvement includes:
Committee on Agricultural Worker Health, Finance Committee, Health Care for People Experiencing Homelessness, Health Care in Public Housing Task Force, health Policy Committee, Legislative Committee and Rural Health Committee.
I am now currently the President and CEO for Community Health Partnership of Illinois, an FQHC with humble beginning as not only a voucher program but one of few health centers that have designated funding for Migrant Seasonal Agricultural Farmworkers that served rural and other communities.
Equipped with a Master’s Degree in Accountancy, more than nine years servicing as President and CEO for now, two Federally Qualified Health Centers and more than 22years in serving health centers in various leadership positions, I know that I can add value to the strategies needed to ensure the financial security of NACHC.
I will use my skills to advance NACHC’s mission, which is dear to the heart of people across States and organizations, supporting meaningful decisions around sustainability.
Clinican Rep: Nicholas Yagoda, MD

In addition to my clinical and executive leadership background, I bring a unique perspective shaped by leading one of the nation’s largest community health systems through two periods of extraordinary disruption—first during the COVID-19 pandemic and more recently as Interim CEO following an abrupt leadership transition. In both cases, I helped stabilize our workforce, rebuild trust, and drive meaningful progress, including restoring financial sustainability, averting layoffs, and expanding access for the underserved.
Beyond crisis response, I have laid the groundwork for CommUnityCare’s emergence as a high-performing, equity-driven organization. I helped elevate our clinical quality performance into the top 20% of FQHCs nationally, led a complex EHR implementation across 29 sites, established a nationally recognized employee wellness program, and built the operational infrastructure for value-based care.
As the leader of a large FQHC poised to have a substantial footprint in value-based care, I would also bring to the NACHC board a critical and timely vantage point: how FQHCs can and should become more integrated into the broader health system landscape. This perspective is vital to achieving NACHC’s bold ambition to reach one in three Americans. It requires not only scale, but also systems thinking, collaboration across sectors, and a commitment to reshaping healthcare around community needs.
I believe in leadership that solves immediate challenges while setting bold aspirations—and building the capacity to meet them. I would be honored to bring that approach to NACHC’s work on behalf of health centers nationwide.
Health Center Board Member Representative: Stan McKee

As a health center patient, current Health Center Board Member Representative to the NACHC Board, and long-time board member of Community Health Service Agency, Inc. dba Carevide in Greenville, Texas, I have spent the last 25+ years supporting the health center movement. I am a retired Senior Manager for Customer Relations at Oncor, the largest electric utility distribution company in Texas. My career focused on serving others which aligns with the mission of Community Health Centers, and I continue to advocate on behalf of patients across the nation to ensure that Legislators are informed about the needs and issues of the patients of Community Health Centers. In addition to serving on multiple committees with NACHC, I also serve on the Texas Association of Community Health Center’s (TACHC) Statewide Advocacy Taskforce and actively participate in NACHC and TACHC conferences each year. I work to engage board members from other health centers across the state and nation to support the strength and survival of health centers for the millions of patients who rely on them.
I graduated from Texas A&M University-Commerce (TAMU-C) where I earned a Bachelor of Science and a Master of Science in Engineering. In addition to my work with Community Health Centers, I am involved with the TAMU-C Alumni Association where I previously served on the Board of Directors. I also participate in activities at my church where I serve as a Deacon at Mt. Moriah Temple Baptist Church.
It would be my privilege to continue serving as the Health Center Board Member Representative to the NACHC Board. I believe my education, commitment, and experience in the health center movement provide a solid foundation to continue serving as your Board Member Representative to the NACHC Board
Health Center Board Member Representative: Malikah Muhammad

My interest in consumer health stems from a concern for affordable, quality healthcare for all Americans. As a consumer, I believe in good health, nutritional foods, exercise, and regular doctor visits. Healthcare is a necessity that impacts everyone, regardless of geographic location or worldview.
Currently, I am NACHC Certified in Health Center Governance. I have been attending the Policy & Issues Forums since 2017 and officially joined NACHC in 2019. At present, I serve nationally on the Consumer, Legislative, and Membership Committees. Locally, I serve on the Patient Experience, Nominating, and Compliance Committees for The Wellness Plan Medical Centers.
I have 31 years of teaching experience and am a retired schoolteacher from Detroit Public Schools. I also serve as an Election Chairman in Detroit (Precinct 150), am a Certified Consultant with over 40 years of experience, and am known for being able to foster peace in emergencies.
The medical staff at The Wellness Plan have taken diligent care of me. My doctors are available for personal visits, emails, or phone calls. Prescriptions are readily accessible, and necessary forms are completed promptly.
Now, I travel across America and see healthcare in action. Millions of Americans are utilizing these services. As a Health Center Board Member, I help oversee the monthly activities of our center, including staff, finances, and daily operations. Each month, we analyze departmental reports.
I am one of the last recipients of the Certificate of Health Center Governance. Attending NACHC governance classes has been the high point of my involvement—a truly rewarding educational pursuit.
If I am honored to serve as the NACHC Consumer Board Representative on the Board of Directors, I will do my best to advocate for the rights of our consumers.

Health Center Board Member Representative: Wes Sternenberg

My name is Wes Sternenberg, and I am honored to be a candidate for Board Member Representative at NACHC. I currently serve as Finance Chair of Albany Area Primary Health Care, a position I have held for the last 10 years, and am a proud patient of the center. In 2023, I was honored to be recognized by the Georgia PCA as their Board Member of the Tear. I also serve on NACHC’s Consumer/Board Member Committee and the Subcommittee on Healthcare Finance, where I advocate for Community Health Center (CHC) board members and contribute insights on key issues affecting CHC’s.
My involvement with NACHC began nearly a decade ago, attending conferences that sparked a deep commitment to the mission of providing high-quality, comprehensive care for all. Since joining NACHC committees in 2020, I’ve served as a presenter and panelist at multiple meetings and workshops, further strengthening my engagement with NACHC.
With 28 years as a CPA specializing in assurance, reimbursement, and consulting for nonprofit healthcare organizations, I lead my firm’s CHC practice and bring extensive experience in addressing the complex issues facing health centers today. I’ve also held leadership roles on various nonprofit boards, which have honed my skills in governance, collaboration, and strategic planning.
I am known for my integrity, dedication, and deep knowledge of the healthcare industry. Guided by a servant leadership philosophy, I am committed to fostering a collaborative, inclusive environment that advances NACHC’s mission.
