Announcing the 2025 Best and Promising Practice Awardees
NACHC’s Elevate Program announced a national call for ‘Best and Promising Practices’ driving transformation across the health center system. More than 100 health centers submitted applications for consideration, with 27 health centers selected for Best and Promising Practice Awards.
These awardees stood out for their outstanding work in areas of health center systems change as outlined in the Value Transformation Framework (VTF) – Care Delivery, Infrastructure, and People systems – demonstrating the positive impact of health centers across the nation.
Transform with Elevate Share your story. Your impact.
Each quarter, the Elevate program will select health centers and PCA/HCCNs who demonstrate how they are successfully using Elevate trainings, tools, and resources to advance their value transformation goals. Selected participants will be eligible for recognition awards ranging from $5,000–$15,000.
NACHC’s Elevate national program is looking for examples of how health centers, PCAs, and HCCNs are using Elevate to support improvement and transformation efforts.
